The following information is required for all public records requests:
- The date the request is being made,
- Name of person requesting records,
- Physical address,
- Email address,
- The business or organization represented, if applicable,
- Whether the request is for commercial purposes, (Additional forms are required for commercial requests. Contact City Clerk Lisa R. Dunbar at [email protected].)
- Statement regarding residency (KRS 61.870(10)a.-g.), (10) "Resident of the Commonwealth" means:
- (a) An individual residing in the Commonwealth;
- (b) A domestic business entity with a location in the Commonwealth;
- (c) A foreign business entity registered with the Secretary of State;
- (d) An individual that is employed and works at a location or locations within the Commonwealth;
- (e) An individual or business entity that owns real property within the
- Commonwealth;
- (f) Any individual or business entity that has been authorized to act on behalf of an individual or business entity defined in paragraphs (a) to (e) of this subsection; or
- (g) A news-gathering organization as defined in KRS 189.635(9)(b)1.a. to e..
- Acknowledgment that fee for duplication must be paid prior to receiving records. (An invoice will be emailed or mailed to the requester for payment.)
- Description of records being requested: If the document name is unknown, provide a brief but specific description of the document or information requested (note date of issuance and location of the document, if known). A request, which is broad, vague, or too voluminous, may cause a delay or denial of your request, and

Applicants for the inspection of public records shall be advised of the availability of the records requested for inspection, and shall be notified in writing, not later than five working days after receipt of an application for inspection, of any reason the records requested are not available for public inspection.
COMMERCIAL REQUESTS - Under the City of Maysville Code of Ordinances Chapter 30:
A certified statement from the requester shall be required for public records intended for use for commercial purposes, and may require the requester to enter into a contract with the City. A fee will be charged for those records based on the cost of reproduction and staff time required to produce the copies.
It is unlawful for any person to obtain records for commercial purposes without stating the commercial purpose, for a different commercial purpose than stated, or for a noncommercial purpose if used for a commercial purpose,
COMMERCIAL PURPOSE is defined as, the direct or indirect use of any public record or records, in any form, for sale, resale, solicitation, rent, or lease of a service, or any use by which the user expects a profit either through commission, salary, or fee.
“COMMERCIAL PURPOSE” shall not include publication or related use of a public record by a news-gathering organization (as defined under KRS 189.635(8)(b)1.a. to e); use of a public record by a radio or television station in its news or another informational program; or use of a public record in the preparation for prosecution or defense of litigation, or claims settlement by the parties to such action, or the attorneys representing the parties.
Requests for commercial purposes must be accompanied by a Certification of Commercial Purpose. Payment for requested document replication and postage, if applicable, must be paid prior to documents being released.
